Title of Spot
Length
Approximate Date Written

Also include the following information about yourself, including:

Full Name
Agency
Agency Address
Position/Title
Work Phone and Fax Number

Once the information is complete, use your email program "file attachment" feature to attach and mail your electronic document to: Contest@oinkradio.com. To mail or fax your form download this PDF document. Fill it out. Then mail or fax the form along with your submitted scripts to:
